To ensure that all of our players enjoy a pleasurable baseball experience here in Farmingdale, and out of concern for their overall safety and well-being, Farmingdale Baseball League, Inc. requires that ALL persons interested in volunteering their time, or to manage/coach a team MUST complete and provide this MANDATORY information form. This may be done in person at registration, at the Shack, or here online electronically.

Under NO circumstances will anyone be granted permission by Farmingdale Baseball League, Inc. to manage and/or coach any team within our boundaries unless and until we have this information on file and on record.

As a condition of volunteering, I give permission for Farmingdale Baseball League, Inc. to conduct a background check on me, which may include a review of sex offender registries, child abuse and criminal history records. I understand that, if appointed, my position is conditional upon the league receiving no inappropriate information about my background. I hereby release and agree to hold harmless from liability Farmingdale Baseball League, Inc., its officers, its directors and volunteers thereof, or any other person or organization that may provide such information. I also understand that, regardless of previous appointments, Farmingdale Baseball League, Inc. is not obligated to appoint me a volunteer position. If appointed, I understand that prior to the expiration of my term, I am subject to suspension by the President or League Director, and removal by the Board for violation of League policies or principles.
